PCORI fees

The PCORI fee is a small annual per-person fee that funds health outcomes research. For self-funded plans and HRAs, the employer files and pays it.

What is the PCORI fee?

The Patient-Centered Outcomes Research Institute (PCORI) fee is an annual fee set by the Affordable Care Act. Carriers pay it for fully insured plans. Sponsors of self-funded health plans, including level-funded plans and most HRAs, pay it themselves by filing IRS Form 720 by July 31, based on the average number of covered lives and a per-life rate the IRS updates each year.

Frequently asked questions

Do fully insured plans pay PCORI?

The carrier pays it. Employers with fully insured medical plans owe PCORI only for a separate self-funded plan, such as an HRA.

Is the PCORI fee paid every year?

Yes, on IRS Form 720 by July 31, using the per-life rate for the plan year being reported.

Where do I find the current rate?

The IRS announces the adjusted per-life amount each year. We confirm it when we help you file.
